Introduction
Patient Journey

We at VericiDx would like to thank the American Kidney Fund for their partnership in this project, and their assistance in providing patient and healthcare professional videos. The videos in the patient journey are meant to bring real life experiences to share with anyone who is searching for information to help them with their own personal journey.

Throughout the journey there will be white tabs with a patient name. Click on these to see videos of patients personal experiences with their journey.

Diagnosis of Kidney Disease

This helps patients to recognize the symptoms, understand what resources are available to help manage kidney disease, to hopefully prevent progression to kidney failure, and if that isn’t possible, what to expect when the kidneys are failing.

Emergency Room Diagnosis

Patients will likely be admitted to the hospital to begin treatments to help them feel better. The process to set a patient up with a general nephrologist and possibly a surgeon may be done here. If a patient doesn’t have a primary care provider (PCP) that will also be set up if possible.

eGFR below 15 ml/min

  1. Long standing kidney disease usually leads to kidney failure (CKD-5).
  2. Patients will have 3 choices at this time, dialysis, palliative care or pursuing transplant. (see buttons below)
  1. - Patients will require dialysis at this point.
  2. - Patients may be transplanted prior to dialysis if they have a living donor.
  3. - Occasionally patients can receive a kidney transplant from a deceased-donor before needing dialysis.
